I wouldn't say it's permanently altered in a negative way. The local ecosystem adapted quite well, and the event led to much stricter surveying protocols for the entire region.

Systemic failure is a polite way to put it. Why isn't there more focus on the fact that they literally created a whirlpool in a lake? Is it really a mapping error when you are drilling blindly into a salt dome?
